Really, I'm pretty much afraid of dying in any way (nuclear war included), but I have a special place for my fear of not being able to breathe. I think it comes from having asthma as a child. It also makes me hate confined spaces.
Also, I'm deathly afraid of spiders. If I see one, I usually have to stand on a chair, away from any walls.
